At your first consultation with Mr Balakumar we will ask you to complete a new patient registration form and medical questionnaire. This is available to download from the website below to save you time when you arrive.

All patients who attend Mr Balakumar will be assessed completely. Assessment includes history, examination, evaluation and investigations to confirm the diagnosis.
